These Forms Terms and Conditions & Disclosures (“Form Terms”) apply to all forms made available on the website of the American Muslim Public Affairs Committee (“AMPAC”), including but not limited to forms relating to donations, pledges, memberships, volunteers, employment, events, merchandise, communications, surveys, and general inquiries (collectively, the “Forms”).

By submitting any form on this website, you acknowledge, understand, and agree to the following:

1. Organizational Status and Purpose

AMPAC is a nonprofit organization described under Section 501(c)(4) of the Internal Revenue Code and is organized and operated for social welfare and public advocacy purposes.

Contributions to AMPAC are not deductible as charitable contributions for federal income tax purposes. AMPAC does not issue charitable tax receipts. You are solely responsible for determining any tax consequences and are encouraged to consult your own professional advisers.

2. No Obligation to Accept or Process Submissions

Submission of any form:

  • Does not obligate AMPAC to process, approve, accept, or act upon the submission

  • Does not create any contractual, fiduciary, agency, employment, membership, donor, or volunteer relationship

  • Does not guarantee eligibility, participation, approval, employment, acceptance, or receipt of any benefit

AMPAC reserves the sole and absolute discretion to decline, delay, review, reject, or condition any submission for any lawful reason.

3. Accuracy of Information and Reliance

You represent and warrant that all information you provide is true, accurate, complete, and current to the best of your knowledge.

You acknowledge that:

  • AMPAC may reasonably rely on the information you submit

  • AMPAC is not responsible for errors, omissions, or misrepresentations supplied by you

  • AMPAC may request additional information or verification before taking action

4. Communications Consent

By providing contact information, including email address or telephone number, you expressly consent to receive communications from AMPAC in accordance with applicable law, including informational, advocacy-related, administrative, or transactional communications.

You may opt out of certain communications at any time using unsubscribe mechanisms or by contacting AMPAC, subject to legal and operational limitations.

5. Payments and Third-Party Processing (If Applicable)

Certain Forms may involve payments, pledges, or financial information.

You acknowledge that:

  • AMPAC does not directly store full payment card information

  • Payments may be processed by third-party payment processors subject to their own terms and privacy practices

  • AMPAC disclaims responsibility for errors, delays, chargebacks, network failures, or actions attributable to third-party processors or financial institutions

AMPAC reserves the right to:

  • Decline or refund payments to the extent permitted by law

  • Require additional verification prior to processing

  • Delay processing for compliance, security, or administrative review

6. Privacy and Data Use

Information submitted through Forms is collected, used, stored, and disclosed in accordance with AMPAC’s Privacy Policy, which is incorporated herein by reference. AMPAC makes commercially reasonable efforts to safeguard information but does not guarantee absolute security. Submission of information is at your own risk.

7. Employment, Volunteer, and Participation Disclosures

Submission of any Form relating to employment, volunteering, or participation:

  • Does not constitute an offer, promise, or guarantee

  • Does not create an employment relationship

  • May be subject to background checks, eligibility criteria, or additional agreements where permitted by law

8. Intellectual Property and Submissions

Unless otherwise stated, any materials, content, or information you submit through Forms may be reviewed, retained, or used by AMPAC for lawful organizational purposes. You represent that you have the legal right to submit any materials provided and that such submission does not violate the rights of any third party.

9. Disclaimer of Warranties

ALL FORMS AND FORM-RELATED SERVICES ARE PROVIDED “AS IS” AND “AS AVAILABLE.” AMPAC DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF ACCURACY, COMPLETENESS, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT, TO THE FULLEST EXTENT PERMITTED BY LAW.

10. Limitation of Liability

To the fullest extent permitted by law, AMPAC shall not be liable for any direct, indirect, incidental, consequential, special, or punitive damages arising out of or relating to:

  • Your submission of any Form

  • AMPAC’s reliance on submitted information

  • Any delay, refusal, or decision regarding a submission

  • Use or misuse of submitted information outside AMPAC’s reasonable control

Where liability cannot be excluded, it shall be limited to the minimum extent permitted by law.

11. Governing Law and Jurisdiction

These Form Terms shall be governed by and construed in accordance with the laws of the District of Columbia, without regard to conflict-of-laws principles. Any dispute not subject to arbitration under AMPAC’s Terms and Conditions shall be brought exclusively in the state or federal courts located in Washington, D.C.

12. Incorporation by Reference

These Form Terms incorporate and operate in conjunction with AMPAC’s:

  • Privacy Policy

In the event of any inconsistency, AMPAC’s Terms and Conditions shall control.

13. Modifications

AMPAC reserves the right to modify these Form Terms at any time. Changes are effective upon posting. Continued use of the website or submission of Forms constitutes acceptance of any revisions.
